Exam Proctoring Center Forms

For Students

• Student Usage Contract
  • Student contract to abide by EPC Rules and UVM Code of Academic Integrity
  • Student must have completed this form to be able to take tests in the EPC

• Exam Scheduling Request
  • Student request to take a test in the EPC.
  • Student must arrange testing, exam delivery ,and return with Faculty
  • Request must be 1 week in advance of test date
  • 2 weeks in advance for Finals

For Faculty

• Exam Proctoring Checklist
  • Faculty Exam Proctoring Conditions
  • Can attach 2 files, e.g., the exam and 1 supporting file
  • Replaces paper form
