Confidentiality Form for the Dean of Students Office (PDF)
All student records are confidential. If a student wishes to give someone access to their record held in the Dean of Students Office, they may fill out this form. This form will not apply for academic or judicial records.

Spouse & Partner ID Form
A university ID card may be issued to a spouse or civil union partner and will be given privileges such as the use of the gym facilities on UVM Family days and nights, same entry fee as students at home sporting events, use of Bailey-Howe Library, and they may open a CAT$cratch Debit Account.

Study Abroad Approval Form
Once you have been accepted into a program, approval for overseas study must be obtained by completing the UVM STUDY ABROAD APPROVAL FORM (SAAF). Learn more about the steps in the process here: Study Abroad 101: The Study Abroad Approval Form
