Welcome to new UVM Students!
When completing your on-line New/Transfer Student Immunization Form, please have available:
* Your UVM Net ID and password
* A copy of your immunization records from your physician or school
A hard copy of your immunization record,
signed by a health care provider, must be mailed to the UVM Center
for Health and Wellbeing, 425 Pearl Street, Burlington, VT 05401 no later than August 15th.
When
completing your on-line Family and Personal Health History Form, please
have available (in addition to your UVM Net ID and password):
* Resources to assist in answering detailed family and personal health history
Please allow at least 30 minutes to complete this comprehensive form in its entirety.
You
must complete the on-line Insurance Waiver Form whether or not you
elect the UVM coverage. To waive/decline UVM insurance, you will
need the name and subscriber ID number of your insurance plan. Please
print the confirmation page for your records.

you have never been immunized due to philosophic or religious beliefs,
or for medical reason (documented by a health care practitioner), you
may submit the exemption form below. This form must be signed by
the student. Note that in the event of an outbreak of an
immunization-protectable disease for which you have not been
vaccinated, the University can require that you be excluded from campus
community activities (i.e., classes) for a period to be determined by
the Vermont Health Department. The printable exemption form below
is acceptable in lieu of the required immunization form.
