If you are a student who would like to request a housing modification based on a non-ADA medical condition, please read this information and have a current primary care/other medical professional complete the following form in its entirety. Student Health Services will review your request and respond directly to you and Residential Life with a recommendation. Residential Life will review the recommendation and issue a housing assignment. In most cases, Residential Life follows our recommendation, but they are sometimes limited by room type availability.
Student Health Services supports the mission of Residential Life, and strives to help all students be successful at UVM. We are happy to be part of your care team and look forward to helping you live comfortably on our campus. Please be aware that we very infrequently recommend single rooms, and there are other modifications that we recommend, such as living in a quiet hall.
To accurately assess your request, we require current and comprehensive documentation of your health condition from a current primary care/other medical professional who is qualified to make the diagnosis. The documentation must substantiate a diagnosed condition as it relates to housing needs. Submission of this form does not guarantee the fulfillment of a request, and this form must be submitted each year for review to renew the request. A note from a doctor which says "I recommend a single room for this student" is not sufficient.
For full consideration, first year students must submit their request by June 30.
