If your intent is to waive or request substitutions for any of the requirements which appear on the Graduation Status Notice using the Request Form process, please list below. Copies of the Request Form are on the web or can be picked up in 528 Waterman. Be aware that waiving a requirement does not reduce the credits required for program completion.

Note: You will not be eligible for program completion unless you successfully complete all requirements as listed on your Graduation Status Notice or have all requests (e.g. course substitutions) approved by the CESS Student Affairs Committee.
