If you wish to complete an IDIMC for your major concentration you must fill out the form below, which is automatically submitted to the Student Services Office. Please note that this is the first step of the process to be eligible for an IDIMC. The second step is to work with your advisor to complete the written narrative rationale. Refer to the guide questions. Once you complete the written rationale you or your Advisor need to give it to the Program Coordinator. He/She will send it to the Student Services Office in 528 Waterman once a program decision has been made.
