Instructions for Applying to the ISE Program at UVM's CDCI

Please note that the application located on this webpage is only for the ISE program. Applicants will also have to fill out the Continuing Education Application located at http://learn.uvm.edu/register in order to register of courses at UVM


To submit an electronic ISE Application download and complete this form:

Include with your completed application:

Please submit completed electronic application form and required materials to: ISE@uvm.edu

Electronic forms should be saved as your last name and the word "application".pdf (ex: jonesapplication.pdf) as the file name before being electronically submitted.

Your Statement of Purpose may also be submitted electronically to: ISE@uvm.edu using your last name and the word "Statement" (ex: jonesstatement) as the file name.

For in person submission of your ISE Application:

If you wish to hand deliver your ISE Application and Statement of Purpose, the Center on Disability and Community Inclusion offices' are located on the 3rd floor of Mann Hall on the University of Vermont's Trinity Campus, 208 Colchester Ave Burlington, VT 50405.

  • If you are delivering your ISE Application and Statement of Purpose by hand please still have your Transcripts and Letters of Recommendation set directly to Darren McIntyre using either the previously listed physical address or the program email address ISE@uvm.edu

Additional ISE Application Requirements

Statement of Purpose:

Please provide a statement of purpose that conveys why you are interested in the ISE program. Address how you intend to use this  , and why you feel that you are a good candidate for the ISE program. If applicable please describe your experience working with students who have low incident disabilities including autism, significant intellectual disabilities, orthopedic impairment, and multiple disabilities. (Your Statement of Purpose should be submitted with your ISE Application and be no more then 750 words)

Letters of Recommendation:

Please provide three letter of recommendation, one from each of the following constituencies:

  • Parent of a past or current student with disabilities
  • Co-Worker
  • Current Administrator

The letters of recommendation should address your interactions with students who have disabilities, and should be submitted directly to Darren McIntyre by the writer either electronically to ISE@uvm.edu or by mail to:

Transcripts:

Please provide a transcript from each post-secondary institution you have attended.
