Planning a CDCI Training: The Logistics (2014)
This document was developed to serve as a guide and checklist for faculty and staff planning to do a training (i.e., workshop, webinar, conference, etc.) through the Center on Disability and Community Inclusion. It includes links for planning information, a Cost Calculation Template, a Registration Template (using LimeSurvey) and a training Satisfaction Survey template (using LimeSurvey).

Adobe Connect Webinars and Meetings will now be scheduled using the Training and Event Survey.
Please provide at least two weeks notice when scheduling a Webinar or Distanced Meeting.

Using the CDCI Training Survey Template
CDCI is required to collect pre and post test training data for all ongoing trainings at the center. Please use the documents below to conduct these surveys.

Consultation Agreement Procedures Manual (2012)
This document was developed to serve as a guide for faculty and staff seeking and/or obtaining consultation agreements to be performed through the Center on Disability and Community Inclusion. The types and purposes of consultation agreements that may be sought vary but most will be assessments, training, or technical assistance in the form of direct services or consultations that are client-specific, to another professional and/or to a whole organization.
