Do you have a CAT$cratch account?

If no, please complete the CAT$cratch application and fax it to 656-4528 or
mail it to:

Dudley H. Davis Student Center, 590 Main Street
Burlington, VT  05405-0152

For the purpose of payroll deduction all we require is your
Name, ID number, Signature and check the Payroll Deduction line.

You will then need to complete the steps below at the Catalyst site to start the deduction.
                   
If yes, click on the link to the PeopleSoft, use the Human Resources Login and follow the steps below

Select:

• Self Serve
• Payroll and Compensation
• Voluntary Deductions
• Add Deduction
• Click on the Search botton beside the line for Type of  Deduction
• Choose CATCARD
• Complete the form and submit
