Diplomas are only issued in May by the Registrar's Office. If you do not attend the May Commencement, then your diploma will be mailed to your permanent address on the system. Your transcript will have the Faculty Senate degree meeting date listed, so you should use this as certification of your degree. This is also requested through the Registrar's Office and should have the appropriate date listed a little while after the meeting has taken place.
Please list your email address and telephone numbers where you can be reached if we have questions. As we will be communicating with you by email, please provide the address that you will be checking on a regular basis during the next year.
If your intent is to request substitutions for any of the requirements
which appear on the CATS report,
please list below for review and possible application. Please also include any questions and/or concerns regarding
your requirements for graduation. Please be as specific as possible.
