This form is for official use only by personnel of University Departments requesting parking for conferences or other scheduled events.
It is not intended for students, visitors or other non-affiliates as a method of requesting visitor parking in advance.

You may access the online Visitor Permit Request Form with your UVM NetId and Password. If you have any questions pertaining to your UVM NetId please visit uvm.edu/account or
University Faculty and Staff members wanting their visitors to park in the 109 South Prospect Visitor Lot, and are unable to make arrangements prior to the visitor's arrival through the Visitor Permit Request Form above.
