SSS Application Instructions

1. To join the SSS program, you must complete an application. You only need to do this one time. Once you are accepted into the program, you can use any of our services until you graduate, as long as you are actively working with your mentor.

3. Please complete as much information as you can.
   
   
4. You do not need parent signature or parent income if you do not have access to this information.
   
   
5. Return your completed application to the Learning Cooperative in Living and Learning
   
   
6. Your application will be processed and you can expect to hear a response by email within one week of submitting your application.
