Address Change


Keeping your address up to date is easy with BANNER SIS. Change your local address, your permanent address, and your phone numbers all online through our secure system. You may also change your address in person at the Student Service Center.

To change your address online:

  • Enter your ID and PIN
  • Select Student Records Information
  • Select Update Address Information
You may change your address at any time.

Confidentiality Form


While most information about you is kept confidential, some information is considered “directory information”and may be publicly shared (for a complete list of what is considered "directory information" click here). Use this form to request that we not release any information, including directory information. Be aware that if you choose to make your record confidential we will not be able to verify your enrollment or degree to a third party without your written signature.

You may request that your record be made confidential at any time.

Continuing Education Registration

If you are interested in taking courses at UVM without applying for a degree program, you can do so through our Continuing Education department. Registration is quick and easy online or, if you prefer, a downloadable form is available.

Registration dates vary from semester to semester so be sure to check here for the most current registration times.

Credit by Examination


If you are a degree-seeking sudent you may be eligible to receive credit for a course by taking a special examination and paying the examination fee ($50 per credit hour). This process is available only for specific courses and you must receive approval from your advisor, the department of the course for which you wish to receive credit, and your Dean's Office in order to be eligible. While there are no specific deadlines for credit by examination, you must take the exam during the semester in which you wish to receive the credit. Be sure to be in contact with the department administering the exam to insure that you meet any specific departmental deadlines.

Declare or Change Your Major/Minor/Concentration/College


If you are a current student, you can both declare and/or request changes to your major, minor, and concentrations online. Once complete, your request will be sent to the appropriate Dean's Office for approval.

Admitted students who have not yet paid their acceptance fees can request changes by writing to admissions@uvm.edu.

You may request that changes be made at any time.

Diploma Request


If you would like an additional copy of your diploma you may request one using this form. The fee for each additional copy is twenty-five dollars. Once we've received your request you can expect to receive your diploma in approximately three weeks.

Domestic Credit Pre-Approval Form


Use this form to request the pre-approval of courses you'd like to take at another U.S. College or University. You can request pre-approval for up to five courses from an institution.

Instructor Permission Form

You may be prevented from enrolling in certain courses due to a failure to meet prerequisites, overlapping course times, or course capacity limits. In order to enroll in these courses you must obtain instructor permission. An instructor can either grant you an override online or this form can be completed and brought to the Student Service Center.

To submit this form during the Add/Drop period only the instructor's and student's signatures are necessary. After the Add/Drop period you must also obtain permission from your Dean's Office to change your enrollment.

Late Payment Appeal


Failure to pay your account balance in full by the due date printed on your statement may result in a late payment fee of $250 and a hold on your account. You may appeal your late fee by submitting this form.

Legal Name/ID Change


You can update your name or ID on your educational record to reflect a legal change by completing this form and mailing, faxing, or bringing it by our office with two acceptable forms of identification. Acceptable forms of identification include a driver’s license, a marriage certificate, a social security card, a passport, and a court document.

You may request a change of name or ID at any time.

Pass/No Pass // Audit Form
(Grade Mode Change)


During the Add/Drop period you may elect to change the method in which you will be graded to Pass/No Pass or Audit.

If you wish to take a course Pass/No Pass you must obtain the permission of your advisor. Please be aware that non-PEAC courses taken Pass/No Pass can only be used for elective credit.

If you wish to Audit a course you must obtain approval from the instructor and your advisor. Courses which are audited receive no grade and no credit, but the usual tuition will be charged.

Residency Application


Students applying to the University often have questions about their residency classifications for tuition purposes. To read more about the Residency Application process, click here.

Applications for In-State Status are available online or in person, or you may request an application be mailed/e-mailed to you by calling (802) 656-8515 or by sending an e-mail to registrar@uvm.edu.

Ski Pass Verification

Some area ski resorts offer discounts on season passes to full-time students. If you wish to obtain a Ski Pass Verification you can do so at the Student Service Center in Waterman during regular hours.

You must be enrolled full-time to receive a Ski Pass Verification.

Tuition Remission Form

If you are a UVM employee, spouse or civil union partner of an employee, or dependent child of an employee, you may be eligible for tuition remission. To read more about the tuition remission benefit click here.

UVM employees no longer need to complete the tuition remission form in order to receive this benefit. Dependents and Partners/Spouses must complete the form each year.

Withdrawal Form

After the Add/Drop period you are not eligible to drop a course. Instead, if you no longer wish to be enrolled in a course, you will need to withdraw. To read more about the difference between dropping a course and withdrawing from a course, click here.
