Holistic, Compassionate Care

Appletree Bay Primary Care is a nurse practitioner-led practice that provides holistic and compassionate evidence-based care. We see our patients as people first; and understand and treat them within the context of their life circumstances, families, jobs and communities. Our nurse practitioners recognize that people want to be heard by their primary care provider and participate in treatment decisions to restore and maintain good health and wellness.

A Patient-Centered Medical Home

Appletree Bay Primary Care has earned Patient-Centered Medical Home status by the Agency for Healthcare Research and Quality. This means that in addition to providing primary care, we coordinate care for patients in need of services we don’t provide. We also ensure that our patients remain up-to-date on routine screenings and services such as mammography, colonoscopy, required laboratory monitoring, immunizations and physical examinations.

The medical home model recognizes that patients must be active participants in their health care for best outcomes.

 

Expert Clinical Faculty

Appletree Bay nurse practitioners have a wealth of experience, including a combined 198 years of nursing and 102 years of nurse practitioner practice. Our areas of expertise include diabetes, COPD, asthma, hypertension, behavioral health, hyperlipidemia, health promotion and disease prevention, geriatric issues, palliative care, and management of health-related transitions in all phases of life. Home visits are possible.

All Appletree Bay nurse practitioners teach in the University of Vermont Nursing program. In the clinic, we work with and mentor registered nurses who are in graduate school to become nurse practitioners. Undergraduate nursing students interested in primary care are also involved at Appletree Bay.

Open Monday through Friday from 8 a.m. to 5 p.m.

Primary Care Services

  • Complete physical and pre-op exams
  • Gynecologic care
  • Acute and chronic illness care
  • On-site blood draws
  • Office procedures
  • ECGs
  • Pulmonary function testing
  • Routine urinalysis and onsite lab tests
  • Referrals for imaging, specialty care, physical therapy, and mental health and substance abuse treatment
  • Health behavior change counseling, including tobacco cessation, nutrition, and exercise counseling
  • Nebulizer treatments for respiratory problems
  • Completion of health-related paperwork, including disability and FMLA documentation
  • After hours, on-call advice for urgent medical issues
