Licensing and Credentialing

Specific inquiries regarding licensure should be referred to the state in which you are applying.

When Applying for Licensure or Employment Privileges:

• Complete the Medical Education Verification/Document Request Form (PDF)
• Submit the form along with any additional forms you need completed to the address below.

As dictated by Federal Law, information regarding academic records can only be released upon written consent of the graduate, so be sure to complete and sign our request form. Requests may be mailed or faxed, and you may e-mail a scanned copy of the signed request form; an e-mail request that doesn't include this signed form is not a valid request and will not be honored.

When completing the request form, please type or clearly print all information on the form. We cannot be held responsible for undeliverable documents if the form is illegible.

Transcript Requests

Graduates of the Class of 2003 or later:

• Please request medical school transcripts through the UVM Registrar's Office.

Graduates from the Class of 2002 and earlier:

• Please request transcripts from the Medical Student Education office. The transcript request form is available at the Registrar's Office website.

Replacement Diplomas

• If you need to request a replacement diploma, please complete the Diploma Reorder Form (PDF) and return it to the Medical Student Education Office along with the $50 replacement fee, payable to the University of Vermont. Please note that the lead time for a replacement diploma is approximately six weeks. Mail to:

• These requests should be forwarded to the Office of Graduate Medical Education at Fletcher Allen Health Care.

Summer Anatomy and Pharmacology students from other schools:
