Complete this form and get it to us. Without your permission, we can legally provide only your name, graduation year and degree received. Please note: The Medical Student Education Office does not verify laboratory assistant training or research. Contact the specific department directly.

FOR: Replacement Diplomas

Complete this form and return it to the Medical Student Education Office along with the $50 replacement fee, payable to the University of Vermont. Please note that the lead time for a replacement diploma is approximately six weeks. These must be mailed to:
