Educational Loan Repayment Programs

Please read the program overview and participant eligibility carefully since this is updated by the State of Vermont annually and changes based on the greatest, current statewide workforce shortages and needs.

The 2015 application has two parts: 1) CORE application and 2) Employer Section E.

Both parts must be completed for the application to be complete. Only complete applications will be considered.

Applications must be typed and received by September 8, 2014. It is the applicant’s responsibility to ensure that the application has been received and is complete by the stated deadline.

Instructions

  • Save these PDFs to your computer.
    • To save, open the document. Go to File, “save as”—select the location on your computer.
    • Rename the CORE file. We suggest “2015 PC ELR your last name, your first initial”
    • Rename the Employer SECTION E file. We suggest “Section E your last name, your first initial”.
  • Email Section E to your employer for completion. Section E can be returned by your employer directly to AHEC by September 8. It is the applicant’s responsibility to ensure that Section E is completed and sent to AHEC by the stated deadline.
  • The forms must be completed using the fillable PDF (i.e., must be typed), then print, sign, and mail to AHEC.

Application Tips

  1. Use the TAB button to move from field-to-field.
  2. Save as you complete. You may return to it later.
  3. Gather and print all of your loan/lender verification documentation in advance.
  4. The Practitioner Statement (Section C) is required and can be typed into the form directly or pasted from a Word document, but space is limited.
  5. The Employer Statement is optional and can be typed into the form directly or pasted from a Word document, but space is limited.
  6. Application can be printed using the "print" button on the application or by going to the Adobe Acrobat menu, File, the Print.

 

Vermont Educational Loan Program for Primary Care Practitioners

The Vermont Educational Loan Repayment Program for Primary Care Practitioners is funded by the State of Vermont through the Department of Health, and is administered by the University of Vermont College of Medicine Area Health Education Centers (AHEC) Program. The goal of this program is to ensure a stable and adequate supply of primary care practitioners to meet the health care needs of Vermonters.

Vermont Educational Loan Program for Nurses

The Vermont Educational Loan Repayment Program for Nurses is funded by the State of Vermont through the Department of Health, and is administered by the University of Vermont College of Medicine Area Health Education Centers (AHEC) Program. The goal of this program is to ensure a stable and adequate supply of nurses (LPNs and RNs) to meet the health care needs of Vermonters.

Vermont Educational Loan Program for Dentists

The Vermont Educational Loan Repayment Program for Dentists is funded by the State of Vermont through the Department of Health, and is administered by the University of Vermont College of Medicine Area Health Education Centers (AHEC) Program. The goal of this program is to ensure a stable and adequate supply of dentists to meet the oral health care needs of Vermonters and increase access for the Medicaid population.

Vermont Educational Loan Program for Nurse Educators/Faculty

The Vermont Educational Loan Repayment Program for Nurse Educators/Faculty is funded by the State of Vermont through the Department of Health, and is administered by the University of Vermont College of Medicine Area Health Education Centers (AHEC) Program. The goal of this program is to ensure an adequate supply of nurse educators/faculty to educate the number of nurses needed to provide quality health care for all Vermonters.
