Tutoring Session Checklist

• Before Tutoring: Gather this information every time you make an appointment
  • Complete name, with correct spelling.
  • Student's e-mail address. 
  • Course code (e.g., BCOR) and number (e.g., 11)  
• Every time you meet with a student, also gather this information:
  • Student Identification Number (9-digits) 
  • Course instructor's name - Please ask them to look for it on their syllabus or to check the UVM registrar's page for the course listing.
  • The complete name, spelled correctly, of anyone on campus to whom the student wishes to have the lognote sent.
• After Each Session
  • Offer your student the Online Tutoring Evaluation Form (paper copies and scantrons are available in the Co-op).
  • Submit your hours
• Helpful Reminders Form (PDF) -- to help you keep track of this information.

Other Forms, Downloads, & Links

• Faculty Recommendation Form -- A faculty member, or graduate student who taught you, from the department must recommend you for each course you wish to tutor.
• Tutee Agreement/Tutor Assignment Form -- This form is to be used to establish expectations between a tutor and tutee in the first tutoring session.  The tutee agreement should be signed and collected for each tutee you work with.  The form also contains basic information you will need to complete your lognotes.  
• Group Log -- This form is to be used when tutoring more than one student at time (group tutoring).  This will take place of a regular lognote filled out for individual tutoring.
