Beginning with the May notices we will
be forwarding to the investigators a reminder notice but no form. The
notice contains a web link to our forms page so that you can obtain
a Word version of the continuing review form. Within this reminder we
will notify you if it is a regular or triennial continuing review and
provide you with the numbers of animals, approved and used, to allow
you to fill out Section 4 of the form appropriately. Make sure that
you reference the notice when you are ready to complete the form. You
will also receive a key personnel roster within the notice. The roster
needs to be printed, completed and attached to the completed continuing
review form for submission. If you have any questions about this new
process, contact our office.
