How to Register for Your Flu Shot

The online registration process is optional but it will save you from filling out paperwork in line. Follow these step-by-step instructions:

1. On the registration page, enter your first name, last name, and the UVM password (UVM2012 in UPPERCASE text). Click SUBMIT.
2. Type in your personal information including address, date of birth, etc.
3. Create your login account by entering your e-mail and a password. (Do not use your UVM password.) Click NEXT.
4. Select the desired service (Flu Shot is the only choice). Click NEXT.
5. Answer all of the Medical History/Health Screening questions carefully by clicking Yes or No. Click NEXT.
6. Choose insurance/method of payment option. Click NEXT.
7. Choose your insurance provider (if applicable) and fill in your insurance information.
8. Enter your credit/debit card information. In the event your insurance does not pay for your immunization, your credit/debit card will be charged for the balance. Your card would only be charged if an insurance claim were filed and denied. (This step is a requirement of the Mollen registration system and is not required if you wait and fill out the paperwork at the clinic.)
9. Review and "sign" the consent form by typing your first and last name next to the signature box. Click FINISH.
10. Click the link towards the bottom of the page in order to view and print the confirmation page. You MUST BRING THIS FORM with you to the clinic—or you will need to fill out the regular paperwork at the clinic.
11. Once you have completed the registration process there is NO way to edit your information, so follow these instructions and enter your information carefully.
12. DO NOT begin the registration process unless you plan on completely filling out the form, have a current version of Adobe Reader, and are currently set up to print from your computer.
