The following is a link to the ASP automated Acccommodations Request Form.

**EBOOK REQUEST FORM**

 

If you cannot access the eBook request form, you can send an email to eBook@uvm.edu with the following information for each of the text books you would like converted;

Title, Author, ISBN (found on back cover near barcode) and the Version or Edition number

or

Class Department (ex. BIO) , Number (ex. 001) , Section (ex. a) and Professor (ex. P. Smith)
