Welcome to our Internet scheduling information system. You may schedule via these forms at any time. If you have questions, please contact Lee Devino, (847-2793), at the CRC, Monday through Friday, 6:00 a.m. to 2:30 p.m.

The scheduler is available from 6:00 a.m. until 2:30 p.m. Monday through Friday.   The scheduler will reply back to you by e-mail either to confirm or to suggest an alternative date and time if we cannot accommodate that visit. You will also receive a copy of your schedule request by email for your records.
