Important Information for Applying to the on-campus UVM Graduate Program

1. Important! - Complete only the first 3 pages of UVM Graduate Online Application.
   • Read Key points for Applicants to the UVM CMSI Graduate Program
   • Applicants will submit 3 letters of recommendation and all official academic transcripts via the CSDCAS Application.
     
2. Complete the Common Online Application for Communication Sciences (CSDCAS).
   • Link to the Centralized Application Service Portal Link.
   • Read the FAQ
3. Download the UVM Worksheet for CMSI Prerequisite Courses and ASHA Undergraduate Requirements Word Document, (2 pages)
   • Applicants will need a copy of their academic transcripts to complete this form.
   • Download the form, complete, and return to the Department of Communication Sciences by Monday, February 1, 2010.

   Submission of this form is a requirement of the Graduate Application Process
