The Vermont
State Transition Team is an inter-agency collaboration developed to ensure
that the transition needs of Vermont students are being met.

Objectives:

Promote
active involvement of youth with disabilities in individual planning, decision
making, implementation, evaluation and choices of the transition services
they will receive

Examine
barriers and gaps in services relative to transition planning

Enhance
interagency collaboration, cooperation, and service delivery for youth
with disabilities and their transition from school to adult life
-
Act as a
resource to local core transition teams, students, families, school and
agency personnel to identify optimum services and funding strategies for
youth exiting high school
-
Identify
content and format for training relative to transition services.
