Hazardous Waste Tags: Four part, serial numbered tags required to be used to arrange for disposal of hazardous chemical waste
Hazardous Waste Accumulation Stickers: Yellow stickers for use on waste accumulation containers not yet full.

- HSRF - 221 Photo room, on shelf
- Stafford - Mailroom on south end of second floor
- Marsh Life Sciences - Stockroom in the basement
- Rubenstein Laboratory - near mailboxes
- Cook Physical Sciences - Room 243, the stockroom on 2nd floor
- Colchester Research Facility - on a shelf inside metal cabinet near main office

Daily Safety Management
Chemical Use Planning Form: form available in pdf and Word format designed to help laboratory workers develop safety practices for specific chemicals or groups of chemicals with similar hazards. Required before submittal of protocols for hazardous chemical use with chemicals.
Laboratory Self-Inspection Form : Color Laboratory safety "scorecard" designed to allow laboratories to review safety conditions in the laboratories at least monthly. You can download and print this or e-mail us at esf@uvm.edu and we'll send you a color paper copy in campus mail.
Laboratory Safety Notebook: General outline of a model laboratory safety notebook. The purpose of this notebook is consolidate safety information in a single place for easy review by laboratory workers, internal laboratory safety auditors and government regulators.

Biosafety Materials
Bloodborne Pathogen Program Forms for Hepatitis B vaccinations: The Risk Designation Form provided by the supervisor, filed in the laboratory safety notebook, and submitted to the medical provider when the Hepatitis B vaccine is given. The Consent/Refusal Form for employees to complete before receiving the vaccine.

Emergency Eye Wash/ Shower Flush Log : Color safety calendar designed to track laboratory safet shower and eyewash flushing as well as self inspections over the calendar year. You can download and print this or e-mail us at esf@uvm.edu and we'll send you a color paper copy in campus mail.
Hazardous Chemical Labels : orange labels in a variety of layouts which provide standardized labelling for hazardous chemicals in UVM laboratories. Be sure that all hazardous chemicals are labelled with these or equivalent labels. E-mail us at esf@uvm.edu for copies.
Non-Hazardous Chemical Labels: green labels in a variety of layouts which provide standardized labelling for non-hazardous chemicals in UVM laboratories. Be sure that all non-hazardous chemicals are labelled with these or equivalent labels. E-mail us at esf@uvm.edu for copies.

"Drains to Lake" Sink Sticker: Sticker to be attached to sinks to remind everyone that what goes down the sinks goes into Lake Champlain (after treatment to remove solids and organic material only).
